Teaching a Revision Rhinoplasty today to other Plastic and Cosmetic Surgeons, in the OR and it came to me that Rhinoplasty and Revision Rhinoplasty takes thousands and thousands of cases performing Rhinoplasty for the Plastic and Cosmetic Surgeon to get a real handle on this difficult procedure. Why?

1. The nose is unlike any other body feature that we do Plastic and Cosmetic Surgery…the tissues do not react well to repeated Rhinoplasties and the tissue can be down right unpredictable during the healing phase. It takes thousands of cases for the Plastic and Cosmetic Surgeon to get a handle on the diversity of healing that may occur and to learn ways to deal with this issue effectively.

2. Rhinoplasty is literally a Plastic and Cosmetic Surgery of mm’s….a little off here and there is the difference between a great nose and one that’s mediocre.

3. The other side of this is …more is better, which means that if a little off looks good, more off the top, off the bones, off the nasal tip is better and this my friends is DEAD WRONG! There is an old saying in surgery “the opposite of good is better”...this means that if it looks good, or you have a good result, trying to get a better result may wind up a disaster. Many Revision Rhinoplasties that I perform are for this very reason…some other Plastic and Cosmetic Surgeon went too far…took off too much bone, too much cartilage, rotated the tip too much. Whatever….but it was too far and required a repeat Rhinoplasty and a trip from all over the world to see me in Beverly Hills.

The take home message that came to me this morning as I am directing 2 other Plastic and Cosmetic Surgeons on this Revision Rhinoplasty here in Beverly Hills is the following: “after almost 20 years of performing Rhinoplasty and Revision Rhinoplasty here in Beverly Hills…it is the added wisdom and with this comes the experience of knowing when enough is enough during the Rhinoplasty. Said another way… it’s knowing when to be bold and when to be conservative and having the courage and conviction of knowing you’ve made the right decision for the patient….that’s what you get from years and years of performing Rhinoplasty.” Easilly the most technically and aesthetically challenging Plastic and Cosmetic Surgery Procedure done here in Beverly Hills or anywhere around the world.
